Family Practice and Counseling Network, a program of Resources for Human Development, is a network of community health centers providing comprehensive primary care, dental and behavioral health services. The Network consists of three comprehensive health centers in Philadelphia – Abbottsford-Falls Family Practice & Counseling in the Northwest, Eleventh Street Family Health Services of Drexel University in the North, and the Health Annex in the Southwest; one health center in York, PA.—East Side Health Center; and a Philadelphia site that focuses on behavioral health services to children and their families– Rochelle Center for Children’s Clinical Services.
The Network employs a nurse-managed health center model to deliver caring, quality primary health care, behavioral health services, dental care, and prevention programs that integrate services in treating the whole patient. Primary care at the Network is provided by Nurse Practitioners with advanced practice degrees and the expertise to diagnose illness and prescribe medication, to make referrals to specialists, to provide pre-and post-natal care, and to offer a wide variety of other primary health care services. Network services are targeted to residents of public housing and the surrounding communities, and the advisory committee that guides services primarily consists of people from the communities served. The Network has earned local and national recognition as a “best practices” model of health care delivery.
